What information does Dan Zohar, P.A. collect?

You can submit to Dan Zohar, P.A. certain information, including but not limited to, your name, address, email address, telephone and cellular telephone numbers, description of the accident or incident, injuries you sustained in the accident or incident, date of birth, and date of the accident or incident (collectively “Personal Information”). We use a tool called Google Analytics to analyze trends, administer the site, track user movement, and to gather broad demographic information, along with 3rd-party audience data (such as age, gender, and interests) for aggregate use for reporting and donation purposes. The analytics tools we use collect only the IP address assigned to you on the date you visit this site, rather than your name or other identifying information. Although the analytics tools plant a persistent cookie on your web browser to identify you as a unique user the next time you visit this site, the cookie cannot be used by anyone but Google.
